Description:
The subject of the order is the successive deliveries of medicines and specialist dressings in the assortments and quantities specified in the price form constituting Annex No. 2 to the SWZ and in accordance with the draft provisions of the contract constituting Annex No. 4 to this SWZ.
The Ordering Party specified in Annex No. 2 to the SWZ quality requirements relating at least to the main elements constituting the subject of the contract.
The Ordering Party guarantees that the minimum value of deliveries for a given task will be 75% of the maximum value of the Contractor's remuneration for this task specified in price form constituting Annex No. 1 to the contract - does not apply to tasks No. 2, 3, 5, 6, 7.
The goods offered by the Contractor are covered by the subject of the contract it must be approved for marketing and use in Poland or it must have a one-time permit from the Ministry of Health. does not apply to task no. 26 i 27
The contractor should provide the trade name of the offered goods and the EAN code in the price form.
The prices of the offered products may not exceed the refund limits announced in announcements of the Minister of Health on the list of reimbursed medicines, foodstuffs for particular nutritional uses and medical devices. - applies to items included in the list of reimbursed medicines.
