Description:
1. The subject of the contract is the supply of medicinal products for UCK in the assortment, estimated quantity and the requirements specified in Annex 3 to SWZ.
2. The Employer allows for partial offers. The concept of partial offer is understood as individual items set in Annex No.3 to SWZ. The contractor may submit an offer in one or several items.
3. As part of the items marked in the assortment and price symbol (**) in Annex 3 to SWZ, the contracting authority requires that the offered preparation be in the reimbursement catalog used in drug programs.
4.The contracting authority in the assortment and price form determined the packaging method. In items in which there is no such description, the contractor may offer any packaging method with maintaining the quantity required by the ordering amount specified in a given item. The contractor must value the proposed waypackaging with appropriate conversion.
5. Deliveries will take place successively, in quantity and assortment, in accordance with partial orders of the Ordering Party within 2 days from the date of placing the order. The contractor will be obliged to deliver the ordered assortment at his own expense to the hospital pharmacy.
6. The contracting authority requires that the prices of offered medicines be not higher than the official prices, as specified in art. 9 para. 1 and 2 of the Act of 12.05.2011, reimbursement of medication of special nutritional food and medical devices (Journal of Laws 11.122.696), in force for the dayfolding ...
