Description:
The subject of the order in this tender procedure is the supply of medicinal products, including within the framework of drug programs, hereinafter referred to as "goods", in accordance with the type of assortment and quantities specified in the "Assortment and price form" constituting Annex No. 2 to the SWZ and in accordance with the "Form "Offers", constituting Annex No. 1 to the SWZ. The order is divided into 259 parts (packages).
The assortment and price form, in the appropriate columns of individual parts, indicate the quantities that will be ordered by the Independent Public Health Care Center of the University Hospital Clinical No. 2 of the Medical University of Lodz, Independent Public Health Care Center, University Clinical Hospital No. 1. N. Barlicki Medical University of Lodz and the Independent Public Health Care Center Central Clinical Hospital of the Medical University of Lodz.
Everyone one of the Ordering Parties signs a separate contract awarding the order in the scope of quantities specified in the Assortment and Price Form for a given hospital.
