Description:
1. The subject of the order is the supply of medical devices for ECMO procedures for UCK in the assortment and the requirements specified in Annex 3 to the ToR.
2. The Ordering Party requires that the offered assortment constituting a medical device meets the requirements for medical devices contained in the Act ofand April 7, 2022 on medical devices (i.e. Journal of Laws of 2022, item 974, as amended) and in the executive acts issued on the basis of this act.
3. Medical devices must meet the requirements set out in Annex 3 to SWZ. Failure to meet at least one of the requirements set out in Annex No3 to SWZ will result in rejection of the offer.
4. Medical devices referred to in par. 1 The Contractor will be obliged to deliver at his own expense to the Medical Supply Department, ul. Smoluchowskiego 17.
5. The delivery of medical devices of this order will be carried out on the basis of partial ordersh. The maximum delivery date is 6 business days from the date of placing the order by Zając
