Description:
1. The subject of the contract is the supply of medicinal products as part of the UCK target import in the assortment, estimated quantity and the requirements specified in Annex 3 to SWZ.
2. The landlord allows the submission of partial offers. A partial offer is understood as individual itemsestablished in Annex 3 to the ToR. The contractor may submit an offer on one or several items.
3. The applicant does not consent to making a change in Annex 3 to SWZ - the assortment and price form under pain of rejection, excluding the situation referred to below:
the contracting authority agreesto:
- change of the form of the proposed preparations, e.g. from vials to ampoule or an ampoule-syringe and vice versa;
- changing the form of the proposed preparations, e.g. from tablets for coated tablets or capsules or dragees and vice versa;
- change of the size of the packaging after the appropriate conversion of units.They will take place successively, in quantity and assortment, in accordance with partial orders of the Ordering Party within 21 days from the date of order. The contractor will be obliged to deliver the ordered assortment at his own expense to the hospital pharmacy.
5. Among the resources, it requires the prices of offered drugsThey were not higher than the official prices, as defined in art. 9 para. 1 and 2 of the Act of 12.05.2011, reimbursement of medication of special nutritional food and medical devices (Journal of Laws of 2024, item 930), applicable as at the date of submission of offers - ifconcerns.
