1. The subject of the order is to deliver to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101:
1) equipment for electrophysiological tests and ablation,by submitting to the deposit of the Ordering Party, together with the lease system for electrophysiological examinations and ablation, as well as a system for three -dimensional heart mapping - part 1,
2) equipment for performing cryoablation of pulmonary veins and point cryoablation - part 2,
according to quantitative and generic statementsspecified in Annex 2 to the specification of the terms of the contract, hereinafter referred to as "SWZ", meeting the qualitative parameters specified in Annex 2 to SWZ, on the conditions set out in the planned provisions of the contract constituting for part 1 - Annex 4a to SWZ, for Part 2 - Annex No.4b to swz.
2. The offered goods must be new and unused, with the exception of the subject of the lease referred to in part 1 and the subject of the lending referred to in Part 2.
3. The subject of the subject of the contract together with transport and unloading will take place at the cost and risk of the contractor.
