- 1. The subject of the contract is the delivery of the range for the needs of the UCK treatment plant in the assortment, estimated quantity and the requirements specified in Annex 3 to SWZ.
A partial offer is understood as single Packages set in Annex 3 to SWZ. The contractor may submit an offer for one, several or all parts.
3. The applicant requires that the offered assortment meet the requirements for medical devices contained in the Act of April 7, 2022 on medical devices (i.e., Journal of Laws of 2024, item 1620) and in the executive acts issued on the basis of the Act.
Failure to meet even one of the requirements specified in Annex 3 to SWZ will reject the offer.
5. The medical devices of this Orders will be carried out on the basis of partial orders. The maximum delivery date is 6 business days from the date of placing the order by the Ordering Party.
6. Medical outlets referred to in para. 1 The Contractor will be obliged to deliver at his own expense to the medical supply department, ul. Smoluchowskiego 17.
