Description:
1. The subject of the contract is the supply of medical equipment with technical parameters specified in Annex 4A, 4B, 4C, 4D, 4E, 4F, 4G and 4H to SWZ, hereinafter referred to as "equipment", together with the installation and training of the Employer's employees.
2. The subject of this order covers the following parts:
Part 1 - Cardiomonitor (2 pieces)
on the technical parameters specified in Annex 4A to SWZ;
Part 2 - patient lift (1 piece)
on the technical parameters specified in Annex 4B to SWZ;
Part 3 - ultrasound (1 piece)
on the technical parameters specified in Annex number 4c to SWZ;
Part 4 - hemodynamic monitor (2 pieces)
on technical parameters specified in Annex 4D to SWZ;
Part 5 - Tromboelastometer (1 piece)
on the technical parameters specified in Annex 4E to SWZ;
Part 6 - Video course (1 set)
About parameters technical specified in Annex 4F to SWZ;
Part 7 - electrostimulator (1 piece)
on technical parameters specified in Annex 4G to SWZ;
Part 8 - pain monitor (1 piece)
on technical parameters specified in Annex 4H to SWZ;
3. The above -mentioned equipment must Meet the border conditions set out in Annex 4A, 4B, 4C, 4D, 4E, 4F, 4G and 4H to SWZ. Failure to meet at least one of the border conditions specified accordingly in Annex 4A, 4B, 4C, 4D, 4E, 4F, 4G and 4H to SWZ will reject the offer.
