Description:
1. The subject of the order is the delivery to the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101:
1) equipment for performing electrophysiological tests and ablation, by depositing in the Ordering Party's deposit, together with the lease of the system for electrophysiological tests and ablation andemu for three-dimensional mapping of the heart - Part 1,
2) equipment for cryoablation of pulmonary venous orifices - Part 2,
3) equipment for performing electrophysiological tests and ablation and cryoablation of pulmonary venous orifices - Part 3,
according to the quantitative and type specification specified in Appendix No. 2 toSpecification of the Terms of Purchase, hereinafter referred to as "swz", meeting the quality parameters set out in Appendix No. 2 to the GTCS, on the terms set out in the draft provisions of the contract constituting for Part 1 - Appendix No. 4a to the SPZ, for Parts 2-3 - Appendix No. 4b to the SPZ .
