Description:
1. The subject of the order is the supply of medical equipment with technical parameters specified in Annex No. 4a, 4b and 4c to the SWZ, hereinafter referred to as "equipment", along with installation and training of the Ordering Party's employees.
2. The subject of this order includes the following parts:
Part 1 Ultrasound scanner (1 piece)
with technical parameters specified in Annex No. 4a to the SWZ;
Part 2 Pain Monitor (1 piece)
with technical parameters specified in Annex 4b to the SWZ;
Part 3 Electrostimulator (1 piece)
with technical parameters specified in Annex No. 4c to SWZ;
Failure to fulfill even one
of the limit parameters specified in Annexes 4a, 4b and 4c to the SWZ, respectively, will result in rejection of the offer.
4. Subject matter of the proceedings is implemented as part of a targeted subsidy from the Ministry of Health for the implementation of the task in 2025: "purchase of equipment and apparatus" as part of the National Transplantation Program by purchasing equipment and apparatus for anesthesiology and intensive care departments with the highest donation activity in 2024 r.
