Description:
1. The subject of the order is the supply of ethyl alcohol, pharmacy remover and formaldehyde for UCK in the assortment, estimated quantity and requirements specified in Annex 3 to the SWZ.
The term partial offer means individual offers parts specified in Annex 3 to the SWZ. The contractor may submit an offer for one or several parts.
3. The offered medical devices must comply with the Act of May 20, 2010 on medical devices (consolidated text: Journal of Laws of 2021, item 1565) and the implementing acts issued on the basis of this Act.
4. Deliveries will be made successively, in quantity and range, in accordance with the Ordering Party's partial orders within 2 days from the date of placing the order. The Contractor will be obliged to deliver the ordered products to the Hospital Pharmacy at his own expense.
5. Ordering Party The Ordering Party requires submission offers in electronic form. A detailed description (instruction) for submitting offers in electronic form is included in section XIV of the SWZ.
The Ordering Party informs that in the public procurement procedure in question the so-called "reverse order of evaluation of offers" referred to in art. 139 of the Public Procurement Law.
Using the above procedure, the ordering party will first examine and evaluate the offers, and then subjectively qualify the contractor whose offer was rated the highest in terms of lack of grounds for exclusion and meeting the conditions for participation in the competition. postepow...
