- 1. The subject of the contract is to deliver to the Provincial Hospital in Bielsko-Biała at al. Home Army 101, laboratory reagents and useful materials, according to the quantitative and generic statement specified in Annex 2 to the specification of the terms of the contract, hereinafter referred to as "SWZ meeting the quality parameters specified in Annex 2 to SWZ, together with the lease of the analyzers and sorter of the tubes for part 3, meeting the operational and utility parameters specified in Annex 2 to SWZ, on the conditions specified in the designed provisions of the contract constituting the attachment No. 4a to SWZ for Part 1-2 and Annex 4B to SWZ for Part 3.
2. The offered goods must be new and unused, except for the subject of the lease referred to in Part 3.
3. The subject of the subject of the contract together with transport and unloading will take place at the contractor's expense and risk.
4. Dotting Part 3 - The Contractor is obliged as part of the price of the subject of the contract, to transfer and deposit at the Ordering Party's headquarters for the duration of the contract, together with the first delivery of reagents, a refrigerator with a capacity of min. 300 l with the cooling range min. 2-8 C for storing the reagents offered and a centrifugal spinning fan with the possibility of adjustable speed up to 4000 revolutions.
