- 1. The subject of the order is the supply of medicinal products for UCK in the assortment, estimated quantity and requirements specified in Annex 3 to the SWZ. 2. The Ordering Party allows the submission of partial offers. The term partial offer means: individual items set out in Annex 3 to the SWZ. The contractor may submit an offer for one or more items. 3. As part of the items marked in the assortment and price form with the symbol (**) in Annex No. 3 to the SWZ, the Ordering Party requires that the offered the preparation was included in the catalog of reimbursed drugs used in chemotherapy/Catalogue of reimbursed drugs used in drug programs. 4. The ordering party specified the packaging method in the assortment and price form. In positions where there is none such description, the Contractor may offer any packaging method while maintaining the quantity required by the Ordering Party specified in a given item. The contractor must price the proposed packaging method with appropriate calculations. 5. Deliveries will occur successively, in quantity and assortment, in accordance with the Ordering Party's partial orders within 2 days from the date of placing the order, and "cito" delivery within 12 hours from placing the order. The Contractor will be obliged to deliver the ordered assortment to: at your own expense to the Hospital Pharmacy. 6. The Ordering Party requires that the prices of the offered medicines are no higher than the official prices specified in Art. 9 section 1 and 2 of the Act of May 12, 2011 on the reimbursement of medicines and foodstuffs...
