- 1. The subject of the order is the provision of voluntary group life insurance services for employees of the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101, 43-316 Bielsko-Biała and members of their families, hereinafter referred to as "Insurance", under the conditions specified:
1)v this chapter;
2) in Annex No. 2 to the Terms of Reference, hereinafter referred to as "SWZ",
3) the draft provisions of the contract constituting Annex No. 6 to the SWZ.
2. Due to the protection of the confidential nature of the information contained in Annexes No. 2A and 3 to the SWZ, Annex No. 2A to the SWZ - "Information about course of voluntary group life insurance for employees of the Provincial Hospital in Bielsko-Biała and their family members" and Annex No. 3 to the SWZ - "Structure of employees of the Provincial Hospital in Bielsko-Biała and their family members", in accordance with Article 133 section 3 of the Public Procurement Law, no will be made available on the website of the proceedings in a direct and unlimited manner.
