Description:
1. The main subject of the order is the supply of disposable medical devices, 2. Detailed description of the subject of the order including information about the assortment and quantities and its requirements were specified in the price form constituting Annex No. 2 to SWZ. 3. The validity period of the offered subject of the contract cannot be shorter than 12 months from the date of each delivery. 4. The product features declared by the Contractor must be consistent with the actual ones throughout the duration of the contract. 5.Orders must be implemented in accordance with the requirements for the offered products. 6. The subject of the order will be delivered at the Contractor's expense, risk and transport to the Hospital Pharmacy - a warehouse in the ground floor, with the possibility of pallet delivery. 7.On the packaging The unit of delivered products must include the trade name, catalog number, series and expiry date before which the product can be safely used. 8. The invoices issued or the WZ document attached to them must include the name trade name, series, expiration date and product catalog number 9.The offered subject of the order must meet the following legal requirements: •The offered products must be medical devices and must meet the requirements specified in the Medical Devices Act on April 7, 2022 (i.e. Journal of Laws 2022, item 974, as amended).
