- Description:
- The order includes successive deliveries of pharmaceutical products - medicines and other medicinal products (hereinafter referred to as "products" or "subject of the order"). Place of delivery - Apteka Szpitalna - at ul. Zygmunta Starego 20 or Department of Pharmacy - ul. Radiowa 2 in Gliwice. There will be a delivery location each time indicated in the order. The scope of the order is described in Appendix No. 2, i.e. Product and price list. Invoicing will take place on one invoice after each delivery, in accordance with one contract concluded for all parts won by the Contractor. An assortment that is not the subject of the contract cannot be shown in the invoice for a given contract. Due to the software used by the Ordering Party, any interference with the assortment and price form is prohibited. The Ordering Party requests that the assortment and price forms be submitted together with the offer in .xls, .xlsx or other format spreadsheet.
