- The subject of the order includes successive deliveries of single-use medical devices along with transport to the headquarters of the Ordering Party - Szpital Miejski w Gliwice Sp. z o. o. - Hospital pharmacy, ul. Zygmunta Starego 20 or Department of Pharmacy ul. Radiowa 2, or the Supply Department at ul. Kociuszki 29, 44-100 Gliwice and unloading to pharmacy warehouses at the expense of the Contractor. The subject of the order was divided into 16 packages (parts). A detailed description of the products is specified in the description of the subject of the order in the Product and price list divided into packages, constituting Appendix No. 2 to the SWZ. The Ordering Party strives to the delivered subject of the order meets the requirements specified in the Act of April 7, 2022. on medical devices (Journal of Laws of 2024, item 799), meets the requirements specified in other applicable specific provisions and standards relating to the subject of the order and meets the minimum requirements specified in the SWZ.Contractor he must have documents confirming the above and is obliged to provide them to the ordering party upon each request and at any time.
