Description:
1. The subject of the contract is the supply of medicinal products used as part of chemotherapy and drug programs.
2. A detailed description of the subject of the contract containing information about the assortment, quantities
and its requirements is specified in the assortment and price form constituting the attachmentNo. 1 to SWZ, in which the contracting authority requires the commercial name of the drug.
3. The validity of the offered subject of the contract cannot be less than 6 months
from the date of each delivery.
4. The offered subject of the contract must be approved for the marketing of medicinal products in accordance with the provisionsOf the Act of September 6, 2001 - Pharmaceutical Law (i.e., of 2016, item 2142, as amended).
6. Applicant requires that medicinal products offered by the Contractor be placed in the medicine catalogreimbursed used in chemotherapy or catalog of reimbursed drugs used in drug programs (compatibility of EAN codes), in accordance with the announcement of the Minister of Health issued on the basis of art. 37 para. 1 of the Act of May 12, 2011 on reimbursement of medicines, special foodsnutritional purpose and medical devices - J.T. Dz. U. of 2015, item 345, as amended died The above provision does not apply to part 1 (item 10,11), 2 items 2.13 items 12.23 and 24
