1. The subject of the contract is the benefit for the provincial hospital in Bielsko-Biała, located at al. Armii Krajowej 101, 43-316 Bielsko-Biała, Transport services, including:
1) providing patients' sanitary transport services for the Provincial Hospital in Bielsko-Biała-Part 1,
2)Sanitary transport services of the biological material and ordering personnel for the Provincial Hospital in Bielsko-Biała-part 2,
3) providing patient transport service for hemodialysis and kidney transplant surgery for the Provincial Hospital in Bielsko-Biała-part 3.
2. TaxThe scope of the subject of the contract is specified in:
1) Description of the subject of the contract constituting Annex 2 to the specification of the terms of the contract, hereinafter referred to as "SWZ",
2) The provisions of the contract for each part of the contract constituting Annex No. 6.1-6.3 to SWZ.take the contractor at the expense and risk of. Armii Krajowej 101, 43-316 Bielsko-Biała, Servicestransport, including:
