Brief description of the subject of the order: 1. The subject of the order is the provision of access services to various sports and recreational facilities and activities, financed from the Company's Social Benefits Fund, in the form of a sports package as part of a monthly subscription, by issuing approx.120 employees of the Ordering Party, free of charge, personalized access cards to activities and facilities., , 2. The access card should entitle the user to unlimited access to sports and recreational facilities and activities in the territory of the Republic of Poland, in particular in the city of Białystok and the Podlaskie Voivodeshipit, without the need to incur additional costs, except for access to facilities and activities indicated by the Contractor as paid or partially paid..3. The Ordering Party allows the possibility of offering cards in the form of: a) plastic personal cards authorizing entry to the premises of a given recreational and social facilitymercury together with presentation of an identity card or, b) magnetic cards or, c) chip cards or, d) proximity cards constituting personal cards enabling entry to individual sports and recreation facilities for each authorized person. 4.
