Description:
The subject of the order is the provision of care services for the charges of the Social Welfare Center in Dobiegniew in their place of residence in the expected number of hours (1 hour 60 minutes): 700 x 11 months = 7,700 hours in accordance with applicable regulations, in especially art. 50 of the Act of March 12, 2004 on social assistance (consolidated text: Journal of Laws of 2025, item 1214, as amended) and the implementing provisions and requirements specified in this SWZ. The Ordering Party reserves the right to outsource the care service place of residence of the residents, but only if this service results from the special health needs of the patients. Its scope will include providing assistance by the caregiver and helping the patient get to medical facilities outside the Dobiegniew commune. A detailed description of the subject of the order is included in Appendix No. 1 to the SWZ. The ordering party will pay for the hours of service actually provided. The Ordering Party reserves the right to limit the scope of the order to 80% of the amount of the service.
