Brief description of the subject of the order: 1. Collection of medical waste with codes: 18 01 02 and 18 01 03 at least every 72 hours from Monday to Friday from 8.00 14.00 and collection of waste with code: 18 01 09 based on written reports from the time point of their storage, i.e. from the Field Station of the Military CenterBlood Donation and Blood Treatment SPZOZ at ul. Kociuszki 30, 19-300 Ek, within 12 months from the date of conclusion of the contract, but not earlier than November 29, 2023, 2. Confirmation of the performance of the collection, transport and weighing of medical waste will be a waste transfer card prepared at BDO in accordance witha from the Act of December 14, 2012 on waste (Journal of Laws of 2023, item 1587, as amended), 3. The Ordering Party informs that the amount of medical waste that is the subject of these proceedings is the amount calculated in estimated method, which may change, decrease or increase during the term of the contractdepending on the actual, current needs of the Ordering Party. However, the value of the concluded contract cannot be changed. , 4.
