1. Name and subject of the order: Construction and equipment of a Day Care Home for Seniors in Wielopole. 2. The subject of the order includes: 2.1. preparation of design documentation, bill of quantities, technical specifications for execution and acceptance of works,2.2. obtaining approval of design documentation in terms of compliance with the functional and utility program,2.3. obtaining all legally required agreements, opinions and decisions necessary to obtain a permit to perform construction works, 2.4. obtaining a decision on a construction permit and/or reporting the intention to perform works on behalf of the Ordering Party, acting on based on the power of attorney granted,2.5. execution of construction works specified in the design documentation prepared by the Contractor and approved by the Ordering Party, 2.6. making all notifications and obtaining all legally required arrangements, acceptances and decisions enabling the commencement of use of the constructed facilities and equipment (including the decision on the use permit or certificate/information on the lack of objection to the use of the facility). 3.
