3 Jeziorna 5, 43-230 Goczałkowice-Zdroj (PL22C)-In the amount of 2,070 ton.
1.2. Water treatment plant Dziećkowice, ul. Wodna 3, 41-107 Imielin (PL22C)-in the amount of 161 tons.
1.3. Station of water treatment for Kozłowa Gora, ul. Leśna 34, 42-584 Wymysłow (PL22B)-in the amount of 23 tons.
2. A detailed description of the subject of the contract is included in Annex 5 to SWZ.
3. DetailedThe terms of the order processing are included in Annex 6 to SWZ.
4. In question:
4.1. In order to demonstrate that the offered product is in accordance with the requirements set out in Annex 5 to SWZ - the description of the subject of the contract, the contracting authority requests that the Contractor submit the following along with the offerin question:
1) Current hygiene certificate issued by the National Institute of Public Health - the National Institute of Hygiene, allowing the product to be used in water intended for human consumption;
2) the technical specification of the offered product.
