The subject of the order is the successive deliveries of disposable materials, medical and non-medical products for "Poddębicki Centrum Zdrowia" Sp. z o. o. in Poddębice, divided into 101 packages in the quantities and assortment specified in Annex No. 2 to SWZ - Assortment and price form. A detailed description of the subject of the order was specified in the Product and Price Form - Appendix No. 2 to the SWZ. The Ordering Party allows the submission of partial offers. The offer may cover the entire item orders or selected parts - packages, and if the package includes more than one item, the offer must be submitted for all items in order to be valid in this package.
