Description:
The subject of the order is: supplies of dressing materials and medical materials for SPZOZ WSS No. 3 in Rybnik.
The delivery specifications are included in Annex 1.1, i.e. "Price forms".
The remaining terms of the order are specified in Annex 2 to the terms of reference, i.e.: Designed provisions of the contract that will be introduced into the content of the contract/contract provisions.
Agreements regarding the resolved packages will be concluded after the completion of these proceedings for a period of 24 months.
The Ordering Party informs that in accordance with art. 139 of the Public Procurement Law, it will first examine and evaluate the offers, and then subjectively qualify the Contractor whose offer was rated highest in a given package in terms of the lack of grounds for exclusion and compliance with the conditions for participation in the proceedings.
Fulfilling the obligation arising from art. 24 section 6 of the Act of June 14, 2024 on the protection of whistleblowers, the contracting authority informs that in the Independent Public Health Care Facility, Provincial Specialist Hospital No. 3 in Rybnik, a procedure has been adopted for internal reporting of information about violations of the law and taking follow-up.
