Description:
The subject of the contract are successive delivery of specialist medicines and dressings in the range and quantities given in the price form constituting Annex 2 to SWZ and in accordance with the planned provisions of the Agreement constituting Annex 4 to this SWZ.
The contracting authority described in Annex 2 to SWZ Qualitative requirements relating to at least the main elements that make up the subject of the contract.
The Employer guarantees that the minimum value of deliveries in the scope of a given task will be 75% of the maximum value of the contractor's remuneration, in this task specified in The price form constituting Annex 1 to the Agreement - does not apply to tasks No. 2, 3, 5, 6, 7.
The goods offered by the Contractor covered by the subject of the contract It must be admitted to trading and for use in Poland or must have a one -off permission of the Ministry of Health. - does not apply to task No. 26 i 27
The contractor in the price form should provide the trade name of the offered goods and the EAN code.
Prices of offered products may not exceed the amount of reimbursement limits announced in announcements of the Minister of Health regarding the list of reimbursed drugs, foods for special nutritional uses and medical devices. - applies to the items in the list of reimbursed drugs.
