Description:
The subject of the contract is the successive delivery of medical equipment- materials for operating procedures for the needs of the Independent Public Health Care Team in Myszkow. The order is divided into 32 packages (parts). Agreements in the scope of individual packages will be concluded after completionof this procedure for a period of 12 months.
The specification of deliveries is included in Annex 1.1 to the specification of the terms of the contract ("price form"). Other terms of the contract are specified in Annex 2 to the specification of the terms of the contract ("Designed provisions of the contract, on the contractpublic, which will be entered into the content of the contract "). All requirements specified in the documents indicated above constitute minimum requirements, and their fulfillment is obligatory. Failure to meet the above -mentioned minimum requirements will result in rejection of the offer as incompatible with the terms of the orderPursuant to art. 226 para. 1 point 5 of the Public Procurement Law Act
The offer consists of the offer form - Annex 1 to SWZ and price form - completed in the range of package/packages on which the offer is submitted - Annex 1.1 to SWZ. The offer should be attached:
