- The subject of the order is the successive deliveries of medicinal products, preparations for enteral and parenteral nutrition, including kits, and glucose tests to the Independent Public Health Care Center in Parczew, divided into 36 tasks. in the range and quantities specified in detail in the attached price forms constituting Annex No. 3 to this ToR. The offered subject of the order must meet the requirements specified in the price forms (Annex No. 3 to the SWZ), in particular in terms of quantity, form, dose and composition. The Contractor will offer an expiry date of the delivered goods of no less than 6 months from the date of delivery. Current deliveries will be carried out by the Contractor in the following modes: 1) regular mode - within 24 hours, from the moment of receiving the order, from Monday to Friday, excluding public holidays (hereinafter referred to as: business days) from 7:00 a.m.-2:00 p.m., except that if the end of the period falls on a Saturday, Sunday or a public holiday, The contractor is obliged to complete the order on the next working day by 2:00 p.m. and 2) "rescue" mode - within 12 hours of receiving the order (24 hours a day, every day of the year) - applies to task no. 1, 2, 3, 4, 5, 6, 7, 26, 28, 31, 32,34. Gross/net unit prices of the offered subject of the contract cannot be higher than the prices resulting from Art. 9 section 1 or 2 of the Act of 12 May 2011 on the reimbursement of medicines,...
