Description:
The subject of the order is the successive deliveries of pharmaceutical products to SPZOZ WSS No. 3 in Rybnik. The delivery specification is included in Annex 1.1 to this Order Specification ("Price Form"). The remaining terms and conditions of the order are specified in Annex 2 to the Terms of ReferenceOrders, i.e. "Drafted contract provisions" that will be introduced into the content of the contract/contract provisions. Agreements regarding the resolved packages will be concluded after the completion of these proceedings for a period of 24 months. The Ordering Party allows for the submission of partial offers, atherefore, the subject of the order is divided into 11 separate parts (packages). The Ordering Party allows the possibility of submitting offers for selected parts (packages), but does not allow the possibility of submitting offers for any selected items in a given part (in a given package), in individual parts (packageh) the full scope provided by the Ordering Party must be valued. The Ordering Party informs that in accordance with Art. 139 of the Public Procurement Law, it will first examine and evaluate the offers, and then subjectively qualify the Contractor whose offer was rated the highest in a given package, in terms of the lack of grounds for exclusion.enia and meeting the conditions for participation in the proceedings.
