Description:
1. Delivery of 2 type B medical ambulances, along with equipment for each ambulance in the form of a main stretcher and a main stretcher transporter. The description of the subject of the order is Annex No. 2 to the SWZ. 2. Name and code of the Common Procurement Vocabulary (CPV): 34114121-3 - Ambulances 33192160 -1 Stretcher 3. The contractor may entrust the performance of part of the order to a subcontractor; in such a case, the Contractor is obliged to indicate in the offer the part of the order whose performance he intends to entrust to subcontractors, and to provide the company (name) of subcontractors, if known at this stage - Annex No. 7 to the SWZ. 4. Date and form of payment - based on issued VAT invoices, by transfer to the Contractor's account indicated in the VAT invoice, within 30 days from the date delivery of the VAT invoice to the Ordering Party. The basis for the Contractor to issue a VAT invoice and request payment of the price is to sign the acceptance protocol of the subject of the contract confirming the execution of the contract in accordance with its terms and conditions, without any reservations; protocol this is an attachment to the invoice. 5. The evidence submitted by all Contractors together with the offer: 1. To confirm that the offered ambulances meet the requirements specified in the SWZ, the Ordering Party requests the submission of the following items: means of evidence: 1) a document confirming the technical data of the ambulance (from the operating manual, vehicle book, factory folder or measured by the Contractor) regarding in particular: - length, width...
