Description:
3.1. The subject of the order is the delivery of accessories for orthopedic procedures - implants within 48 hours of placing the order and providing the instrumentation for SP ZOZ in Łęczna to the operating theater.
3.2. At the headquarters of the Ordering Party, the Orthopedic Accessories Bank will be created - implants constituting the subject of the contract, which will be the property of the Contractor and will be subject to successive supplement by the Contractor.
A detailed description of the subject of the contract can be found in the price form constituting annex 2 attached to SWZ, divided into 6 sets:
Set No. 1 33184100-4 - Surgical implants
Set No. 233184100-4 - Surgical implants
Set No. 333184100-4 - Surgical implants
Set No. 433184100-4 - Surgical implants
IN within The gross values of the set, the contractor is obliged to train 10 doctors and 10 nurses in the extent necessary to gain appropriate knowledge and qualifications, within each set during the time and dates indicated by the Ordering Party during the period of this contract.
Whole The assortment should have permits confirming the marketing admission in Poland.
