Description: 2.1. The subject of this contract is the purchase and delivery of active substances in drug programs for 2021 to the hospital pharmacy at Strzelcze Bytomski 11 in Chorzów, divided into the following packages:
- Package 1 - Active substances used in the drug program in the treatment of patients with chronic myeloid leukemia,
- Package 2 - active ingredients used in the drug program in the treatment of chronic thrombocytopenia,
- Package 3 - active ingredients used in drug programs used in the treatment of patients with acute lymphoblastic leukemia,
- Package 4 - Active substances used in drug programs used to treat patients with chronic myelogenous leukemia.
2.2. A detailed scope of the contract was specified in the assortment and price specification (hereinafter in the following: SAC) constituting Annex 2 to this SWZ.
