Description:
The subject of the order is the delivery of a digital osteoarton and hearing apparatus to the X-ray workshop. The medical equipment offered by the contractor must have current permits for use in Poland - in accordance with the Act of 7 April 2022 on medical devices (i.e., Journal of Laws of 2024, item 1620, as amended. d.) and have relevant compliance declarations (CE certificates).
The subject of the contract includes in particular:
1) supply and installation of a brand new camera with additional equipment and accessories in the rooms indicated by the Employer - according to the minimum border list technical, qualitative, operational and functional parameters specified in Annex 1a to SWZ. Installation of the camera by the manufacturer's authorized service.
2) assembly, launch and testing,
3) conducting trainings of the Employer's personnel,
4) granting a full warranty on supplied device.
5) provision of services in the field of repair and maintenance of devices
A detailed description of the subject of the contract and the method of execution of the order include Annexes No. 1A and 4 to SWZ.
Order carried out under contract No. DOI/FM/SMPL/57/MDSOR/2023/494/294 for granting targeted to finance the investment entitled "Modernization, retrofitting SOR and retrofitting of the TK and X -ray studio" - purchase financed from a targeted subsidy from the medical fund under the program: "Investment program of modernization of medical entities"
