1. The subject of the contract is the provision of services in the field of delivery and transport of children and adolescents from disability (including people in wheelchairs) from their place of residence to schools and specialist education institutions (hereinafter referred to as "educational institutions") and their removal from schools to places of residence, along with providing care during transport, in the school year 2025/2026 and 2026/2027.
2. The Contractor will provide transport services for children and young people with disabilities in accordance with the following statement:
1) delivery of children from places of residence to educational institutions - 96 people
2) delivery of children from educational institutions to places of residence - 96 people
3) Number of learning days Szkolna, in which delivery and passages will be carried out:
a) school year 2025/2026: 189
b) school year 2026/2027: 188
4) The number of vehicles necessary to handle the recommended transport routes: 23
5) total length of transport recommended for implementation:
a) daily maximum 1150 km
b) total maximum 433 550 km
3. The applicant provides for the possibility of exercising the right of options, i.e. the right to reduce or increase the order within the limits of up to 20% of the number kilometers of transport indicated in para. 2 point 5 lit. B above, i.e.
a) guaranteed, minimum scope of the order (basic order), after take into account the right to reduce the order is a transport in the amount of 346 550.4 km,
b) the maximum scope of the order carried out taking into account the right of the option, after taking into account the right to increase orders... - Documents:
