Description:
1. The subject of the order is the supply and installation of intravascular laser athectom, hereinafter referred to as "equipment". Along with the atherectom, the contractor will also provide laser catheters. The quantities of catheters given in the unit price form are indicative and the contracting authority is entitled Order them smaller amounts, except that not less than 80% of the value of the contract for laser catheters.
2. The description of the subject of the contract was included in the "Parameters required" form (Annex 2 to SWZ, hereinafter also called "FPW") and in the unit price form (Annex 2.1. To SWZ, hereinafter referred to as as "FCJ"), these forms should be absolutely submitted with the offer form (Annex 1 to SWZ, hereinafter referred to as "FO").
Delivery and installation of intravascular laser athectomy.
1. The subject of the contract is the delivery and installation of the intravascular laser athectome, hereinafter referred to as "Equipment". Along with the atherectom, the contractor will also provide laser catheters. The amounts of catheters given in the unit price form are indicative and the contracting authority has the right to order their smaller amounts, except that not less than 80% of the value of the contract for laser catheters.
