The subject of the contract is the supply of anti -cancer agents, immunosuppressive agents, immunoglobulins, broken down into 14 parts., In accordance with Annex 2 to SWZ - forms (assortment and price) and the draft contract.from participation in the procedure, submitted to the contracting authority of the contractor, whose offer was the highest rated (within the time limit set by the Ordering Party, not less than 10 days):
1. Information from the National Criminal Register within the scope specified in:
a) art. 108 para. 1 points 1 and 2PZP,
b) art. 108 para. 1 point 4 of the Public Procurement Law, regarding the decision to apply for a public procurement as a penalty, drawn up not earlier than 6 months before its submission.
2) the contractor's declaration, in the scope of art. 1 point 5 of the PZP, about the lack of belonging to the sameCapital group within the meaning of the Act of 16 February 2007 on competition and consumer protection (Journal of Laws of 2023, item 1689), with another contractor who submitted a separate offer, or the contractor's statement about belonging to the same capital group together with documents or information confirmingPreparation of the offer regardless of another contractor belonging to the same capital group - in accordance with the template constituting Annex 4 to SWZ.
