Description:
The subject of the order is the supply of anti -cancer agents, bacterial antifungal agents, various medicinal products, painkillers, immune serums and immunoglobulins, medicinal products for sense organs, medicinal products for dermatology, in district on 85 parts, in accordance with Annex 2 to SWZ - forms (assortment and percentage) and draft contract.
Subjective evidence regarding demonstration of a lack of grounds for exclusion from participation in the procedure, submitted on the request of the contracting authority by the Contractor, whose offer was the highest rated (within the time limit set by the Ordering Party, not less than 10 days):
108 para. 1 points 1 and 2 of the PZP,
b) art. 1 point 4 of the Public Procurement Law, regarding the decision to apply for a public procurement as a criminal measure, prepared not earlier than 6 months before its submission.
