- The subject of the order is the supply of anticancer agents and medicinal products for the cardiovascular system, divided into 4 parts, in accordance with Annex No. 2 to the SWZ - forms (product range and prices) and draft contracts.
Subjective means of evidence for demonstrating the lack of grounds to be excluded from participation in the proceedings, submitted at the request of the Ordering Party by the contractor whose offer was rated the highest (within the deadline set by the Ordering Party, not shorter than 10 days):
108 section 1 point 1 i 2 PZP,
108 section 1 point 4 of the PZP, regarding the prohibition of applying for a public procurement contract as a penal measure, prepared no earlier than 6 months before its submission.
2) Declaration of the Contractor, within the scope of Art. 1 point 5 of the PZP, about not belonging to the same group capital within the meaning of the Act of February 16, 2007 on competition and consumer protection (Journal of Laws of 2023, item 1689), with another contractor who submitted a separate offer, or the Contractor's declaration of belonging to the same capital group along with supporting documents or information preparing an offer independently of another Contractor belonging to the same capital group - in accordance with the template constituting Annex 4 to the SWZ.
