Description:
1. The subject of this order is the delivery of a digital mammography machine along with modernization works for the National Institute of Oncology. Maria Skłodowska-Curie - National Research Institute, Branch in Gliwice. 2. Description of the item orders and terms of order execution are included in the following annexes: 1) Technical parameters - Annex No. 2 to SWZ, 2) Product and price specification - Annex No. 3 to the SWZ, 3) Description of the modernization of rooms - Annex No. 4 to SWZ, 4) Draft provisions of the contract that will be introduced into the content of the contract - Annex No. 9 to the SWZ. 3. Offers that do not include the full scope of the subject of the order will be rejected. 4. The Ordering Party allows the possibility of submitting equivalent offers - the description of equivalence is included in Chapter IV of the SWZ. 5. The Ordering Party does not provide for electronic auctions or supplementary orders. 6. Information on the joint application of Contractors for the award of the contract is included in Chapter IX of the SWZ. 7. The Ordering Party provides for the possibility of changes to the provisions of the concluded contract (so-called contract changes based on Article 455(1)(1) of the Act) in relation to the content of the offer on the basis of which the Contractor was selected, in accordance with the conditions contained in Annex No. 9 to SWZ. The contract may also be changed in the cases referred to in Art. 455 section 1 points 2-4 and section 2 of the Public Procurement Law. 8.
