Description:
1. The subject of this order is the supply of a digital radiography device equipped with tomosynthesis technology for the National Institute of Oncology. Maria Skłodowska-Curie - National Research Institute, Branch in Gliwice.
2. Description of the subject of the order and terms of implementation the orders are included in the attachments:
1) Technical parameters, assortment and price specification - Annex No. 2 to the SWZ,
2) Draft provisions of the contract that will be introduced into the content of the contract - Annex No. 4 to the SWZ.
3. The Ordering Party does not allow the submission of partial offers.
4. Offers that do not include the full scope of the subject matter of the contract will be rejected.
5. The Ordering Party allows the possibility of submitting equivalent offers - the description of equivalence is included in Chapter IV of the SWZ.
6. The Ordering Party requires the Contractor to pay a deposit and secure the proper performance of the contract
7. The Ordering Party does not provide for electronic auctions or supplementary orders.
8. Each Contractor must attach to the offer evidence that confirms that the offered subject of the contract meets the requirements specified by the Ordering Party:
Declaration of conformity and/or certificate CE (depending on the class of the medical device) and appropriate declarations (if required) in accordance with Regulation 2017/745 on medical devices (MDR) or Council Directive 93/42/EEC (MDD) and subsequent transitional provisions.
