Description:
The subject of the contract are successive delivery by the contractor of a one -off assortment for the cardiac surgery department for a period of 36 months, hereinafter referred to as the commodity, described in detail in the content of the price form constituting Annex 2 to SWZ together with the lease of the generator for ablation (tasknoits duration, the Contractor will lend the Employer Holder (hereinafter: the subject of the lending) - after 1 item in each task.
The contracting authority guarantees that the minimum value of the contractor granted during the period of the contract in the scope of a given task will be 75% of the maximum valueThe contractor's remuneration in this task, specified in the price form constituting Annex 1 to the Agreement
The contracting authority informs that in the subject proceedings conducted under an open tender, it provides for the possibility of applying the procedure referred to in art. 139 para. 1 of the ActThe contracting authority, using the said procedure, will first examine and evaluate the offers, and then perform the subjective qualification of the Contractor, whose offer was the highest assessed in the scope of the lack of grounds for exclusion and meeting the conditions of participation in the proceedings.
