Description:
The subject of the order is the delivery of disposable, collectively packed sets and accessories necessary for cataract surgery, along with the lease of a phacoemulsification device - described in detail in Annex 1 to the SWZ.
1) Act of April 7, 2022 on medical devices (Journal of Laws No. 2024, item 1620),
2) REGULATION (EU) 2017/745 OF THE EUROPEAN PARLIAMENT AND OF THE COUNCIL of 5 April 2017 on medical devices, amending Directive 2001/83/EC, Regulation (EC) No 178/2002 and Regulation (EC) No 1223/2009 and repealing Council Directives 90/385/EEC and 93/42/EEC (MDR), as well to have required documents - regarding item 1, 2, 3, 4, 5 listed in Annex 1 to the SWZ.
2) The phacoemulsification device (hereinafter referred to as the "equipment") must be complete and ready for operation after startup (year of production min. 2022), guaranteeing the safety of medical staff and ensuring required level of medical services provided - description of technical parameters in Annex No. 1A to the SWZ.
3) The equipment rental fee will include: all parts and elements necessary for the proper functioning of the equipment, full equipment service, throughout the duration of the contract, min. 1 review equipment for a year (unless the manufacturer recommends more often), delivery of equipment to the Ordering Party's headquarters, commissioning of the equipment, training of the Ordering Party's staff..
