Description:
The subject of this public procurement procedure is the supply of one -off equipment and medical materials for the general hospital in Kolno, in the quantity and assortment specified in Annex 2 to SWZ. Number of packages: 47.
A detailed description of the subject of the contract has been specified in:
• Annex 2 to SWZ-assortment and price form.
Conditions for the performance of the order were specified in the template of the contract constituting:
• Annex 11 to SWZ - contract template.
Contractors may apply for the award of the contract, which there are no grounds for exclusion from proceedings pursuant to art. 108 para. 1 and art. 109 para. 1 point 5), 7), 8), art. 7 para. 1 of the Act of April 13, 2022. on special solutions in the field of counteracting aggression to Ukraine and to protect national security (Journal of Laws of 2024, item 507) and art. 5k para. 1 of Regulation 2022/576 of April 8, 2022. on the amendment of Regulation (EU) No 833/2014 regarding the limiting measures in connection with Russia's actions destabilizing the situation in Ukraine (Journal of Laws of EU No. L 111 of 8.4.2022, p. 1) and meet the conditions below, i.e.
