Description:
1. The subject of the order are the supply of disposable medical materials.
2. A detaching description of the subject of the contract together with the required parameters, as well as quantitative demand, specifies Annex 2 to SWZ - price form.
3. The contractor is obliged to process the order onThe principles and conditions set out in the template of the contract constituting Annex 7 to SWZ. 4. The offered subject of the contract must comply with the requirements of the Ordering Party described in SWZ and the legal provisions in force in Poland. In addition, the subject of the contract must have a CE designation, compliance declarations,approvals, certificates and registration certificates if required by law. In addition, medical devices must have certificates of allowance and use in Polish healthcare facilities.
