Description:
1. The subject of the contract is the supply of disposable products to urological and surgical procedures, lease of 3 electric staplers for the UK-2 PUM in Szczecin.
2. Iir, in the further part SWZ will be used:

a) "products"-this concept should be understood as the products indicated in the attachmentNo. 2 (FCj) to SWZ;
b) "equipment" - it should be understood under this concept of 3 leased electrical staplers from task No. 8, whose descriptions and technical requirements are included in Annex 2 (FCj).
3. The desire for the subject of the contract was included in the unit price form (Annex 2 to SWZ, hereinafter referred to as as"FCJ") and should be submitted with the offer form (Annex 1 to SWZ, hereinafter also called "FO").2022 on medical devices (Journal of Laws 2024.1620, as amended) the marketing and use on the territory of the Republic of Poland in accordance with applicable regulations. Does not apply to task 8 part B.
6. The number of products given in the unit price form is approximate and the contracting authority has the right to order themsmaller amounts, except that not less than 60% of the contract/task value.
