The subject of the order is the successive delivery of the drug used to treat patients with achondroplasia, in the therapeutic program B. 166.

1. The subject of the contract is the successive delivery of the drug used in the treatment of patients with Achondroplasia, in the therapeutic program B. 166., in the quantity and assortment specified in the price form, constituting Annex 2 to SWZ. A drug in three available therapeutic doses.
Drug program reimbursed by the National Health Fund:
B.166. Treatment of patients with achondroplasia (ICD-10: Q77.4)
2. Drugs The subject of the contract must have permission to marketing in accordance with the Act of 06/06/2001 - Pharmaceutical Law (Journal of Laws 2025 item 750) and be in the current "official list of medicinal products allowed to market in the territory of the Republic of Poland or have valid as at the date of submission of offers, current permission to marry a medicinal product issued by the European Union Council or the European Commission based on the legal provisions in force in this respect "(at the request of the contracting authority the contractor is obliged to show up to date registration certificates of provided drugs).
