- 1. The subject of the order is the delivery of an endoscopic set for spine surgery to the Ordering Party's headquarters in accordance with the description and requirements contained in Annex No. 1 to the Specification.
2. Offers that do not include the full scope of the subject of the order will be rejected.
3.Other terms of the order specifies the draft contract, constituting Annex No. 3 to the Specification.
REQUIRED DATE AND CONDITIONS FOR ORDER COMPLETION
1.Deadline for delivery of equipment: up to 8 weeks from the date of conclusion of the Agreement.
2. Delivery and, if applicable, assembly, installation and commissioning of equipment at the Specialist Hospital. Ludwika Rydygiera w Krakowie sp. z o. o., 31-826 Krakow, os. ZΕota JesieΕ 1.
3. On the day of delivery of the subject of the contract, the Contractor undertakes to conduct training for the Ordering Party's staff in the use and sterilization at its headquarters - os. ZΕota Jesieni 1, Krakow or provide your own assistance the cost of such training provided by a third party. The training will be confirmed by issuing personal certificates.
CONDITIONS OF PARTICIPATION IN THE PROCEDURE AND THE METHOD OF ASSESSING THE MEETING OF THESE CONDITIONS
Contractors who meet the conditions specified in Art. may participate in the award of the contract. 57 of the Act Pzp and art. 112 section 2 of the Public Procurement Law, i.e.:
1) are not subject to exclusion
2) meet the conditions for participation in the proceedings regarding:
a) ability to engage in business - the Ordering Party does not set any detailed conditions for participation in the Proceedings.
