Description:
1. The subject of the contract is to provide an endoscopic set for spine surgery to the Ordering Party's headquarters as described and the requirements contained in Annex 1 to the specification.
2. Openses that do not have the full scope of the subject of the contract will be rejected.
3. Conditions of the contract Specifies the draft contract, constituting Annex 3 to the specification.
The required date and terms of the order processing
1. Equipment delivery: up to 8 weeks from the date of the contract.
2. Display and if it concerns the installation, installation and launch of equipment at the Specialist Hospital Ludwika Rydygiera in Krakow sp.z o.o., 31-826 Krakow, os. Golden Autumn 1.
3. The contractor undertakes on the day of delivery of the subject of the contract to conduct training of the Employer's personnel from use and sterilization, at its headquarters - os. Golden Autumn 1, Krakow or assurances for your own The cost of such training by a third party. The training will be confirmed by the issue of personal certificates.
Conditions for participation in the procedure and how to assess these conditions
Contractors who meet the conditions specified in art. 57 of the Act PZP and art. 112 para. 2 of the PZP Act, i.e.
1) are not subject to exclusion
2) meet the conditions for participation in the proceedings regarding:
a) the ability to occur in business transactions - the Employer does not set a detailed condition for participation in the procedure.
