Description:
1. The subject of the order is the delivery of equipment by the Contractor as part of oncological rehabilitation, including:
Task No. 1 Delivery of equipment for oncological rehabilitation Group 1
Task No. 2 Delivery of equipment for oncological rehabilitation Group 2
Task No. 3 Delivery of rehabilitation equipment oncology Group 3
(hereinafter referred to as equipment), described in detail in the List of Required Functions and Technical Parameters constituting Annex No. 6 to this SWZ, in the quantities indicated in the price form constituting Annex No. 2 to the SWZ and in accordance with the Draft Provisions of the Agreement constituting the Annex
1 will be carried out as part of the contract concluded with the State Treasury and the Minister of Health for the implementation of the multi-annual program entitled "National Oncological Strategy" in the field of "Purchase of equipment for therapeutic rehabilitation for oncology patients in 2025" (hereinafter referred to as the subsidy agreement).
